You are hereby notified that on April 4, 2017 petition for the adoption of Elijah James Clocquennoi, a child who was born on July 3, 2009 was filed in Clark County Probate Court; and the consent hearing on said petition will be had before said Court on May 9, 2019 at 10:00 A.M. followed by a best interest hearing to be held on June 10, 2019 at 10:00 A.M. It is alleged in the Petition for Adoption, pursuant to R.C. 3107.07, that the consent of Dustin M. White is not required due to the following: (X) the parent has failed without justifiable cause to provide more than de minimis contact with the minor for a period of at least one year immediately preceding the filing of the adoption petition or placement of the minor in the home of the petitioner. (X) the parent has failed without justifiable cause to provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding the filing of the adoption petition or the placement of the minor in the home of the petitioner.
