LEGAL AD AND NOTICE TO PROPOSERS CONTRACT NO. EAP070819 Sealed proposals will be received at the office of the Butler County Commissioners, Butler County Government Services Center, 315 High Street, 6th Floor, Hamilton, Ohio 45011 until 12:00 P.M. EST on Friday, August 9, 2019. This RFP is for a contract to provide an employee assistance program providing short-term counseling and referral to Butler County employees and their eligible family members, for a one-year period with up to four one-year renewal options at the discretion of the Butler County Commissioners, in accordance with specifications under Butler County Contract No. EAP070819. Specifications may be obtained at the office of the Butler County Commissioners or by query at www.butlercountycommissioners.org. Each proposer shall submit with proposal, a bid bond in satisfactory form in favor of the County in the amount of $1,000.00. A performance bond may be required for the project. The proposer may provide a combination bid/performance bond, which is in statutory compliance with the Ohio Revised Code. Proposers are required to complete and submit with proposal ALL forms that are included with the specifications packet. Proposers are required to use the forms furnished by the County when submitting their proposal. Proposers should submit the original proposal along with five (5) copies. The Board of Butler County Commissioners reserves the right to waive informalities, to reject any and all proposals, to accept the proposal which is in the best interest of the County as determined by the Board, and to hold proposals valid for sixty (60) days from the proposal date. Proposals are to be SEALED and delivered to the office of the Butler County Commissioners, and each proposal shall bear on its face the name and address of the proposer and shall be plainly marked "Proposal on Contract No. EAP070819, EAP Services."
