REQUEST FOR STATEMENTS OF QUALIFICATIONS FOR PROFESSIONAL CONSULTING SERVICES REQUEST FOR STATEMENTS OF QUALIFICATIONS FOR PROFESSIONAL CONSULTING SERVICES To: Qualified consulting firms From: Village of Spring Valley, Ohio Re: Consulting services for Spring Valley Water Tower Replacement Project Date: May 21, 2019 The Village of Spring Valley is requesting statements of qualifications for professional consulting services for the Spring Valley Water Tower Replacement Project. It is the intention of the Village of Spring Valley to review statements of qualifications submitted then interview short-listed firms before making the final selection. Three copies of statements of qualifications (SOQ) should be sent to the Village of Spring Valley, 7 W Main Street, Spring Valley, OH 45370, by 5:00 p.m. on June 10, 2019. 1. REQUIREMENTS FOR STATEMENTS OF QUALIFICATIONS (SOQ) The statement of qualifications should be 5 pages. Statements of qualifications of more than 5 pages will not be accepted. Page 1 Name, address, and brief description of firm. Number of years in business. Identify the specific office that would be handling the project if selected. Identify the project manager who would be working on the project if selected. Page 2 Include the project manager's resume. Page 3 Include contact information (community, name, phone, address, email) for five current projects which the project manager is working on. Page 4 Include three examples, one paragraph each, of similar projects the firm completed. Page 5 Include a one-page narrative describing the firm's interest in the project, particular abilities and qualifications related to this project, and reasons why the firm should be selected as the consulting engineering firm. 2. DESCRIPTION OF PROJECT/ SCOPE OF SERVICES The Village of Spring Valley needs to hire a professional consulting firm to complete a water tower replacement design, bidding, inspection and construction administration. The cost of completing the design will not be considered during the selection process (according to the Ohio Revised Code). The Village of Spring Valley intends to utilize various state and federal programs to finance identified projects and to utilize the services of RCAP in acquiring financing for identified projects. Village of Spring Valley Water Tower Replacement Project Provide engineering services to design a 100,000 gallon steel standpipe for potable water storage, including the necessary sitework, water main tie-in, foundations, structural support, coating, mechanical, electrical, control and instrumentation systems. The consultant will be required to provide construction management and quality control inspections during field installation. Consultant will be required to file permits on behalf of the Village and to coordinate with local, county, state, federal and other public utility agencies as applicable. The consultant's scope of work will generally include the following: 1)The consultant will produce a subsurface geotechnical study of the site prepared by a geotechnical engineer licensed in the State of Ohio. 2)The consultant shall prepare and provide all necessary bid documents and drawings for the tower, foundations, and accessories as follows: All civil, structural, mechanical, electrical and instrumentation drawings. 3)The consultant shall prepare a Project Manual including both General Conditions and Technical Specifications. 4)Provide a preliminary (60% design) and final engineering estimate (100% design) of the construction of new elevated water tank. 5)Manage a pre bid meeting at the Owner's facility. 6)Provide bidding support services including responding to bidder's questions, preparation of addendum, assist in receipt of bids, analysis of bids received, and assist in award of the construction contract. 7)Provide construction administration services including field progress reports, preconstruction and construction meeting minutes, review and approval of submittals, response to RFI's, review of contractor's payment requests, change order review and closeout documentation: punch list, as-builds, O& M manuals. Allow for ten (10) construction meetings which will be held once a month. 8)Provide inspection of water tank installation. The inspectors assigned to the work in the field are required to be experienced NACE Certified inspectors. Experienced welding inspectors are required to be AWS Certified Welding Inspectors working under the direction of an Ohio licensed structural engineer. Inspectors must have a minimum of five (5) years of experience. Submit inspection reports. Allow for ten (10) tank inspections. 9)Coordinate with village staff, county and state other public utility agencies. 3.
