TO: Carolyn Elizabeth Gerakines 1012 Millerton Dr. Centerville, OH 45459 In the Matter of: Carolyn Elizabeth Gerakines, RN On July 23, 2020, the Ohio Board of Nursing (OBN) issued a Notice of Opportunity for Hearing to Ms. Gerakines, mailed on July 24, 2020, via certified mail, to her address of record listed above. Said delivery failed. The Notice states that OBN intends to consider disciplinary action against Ms. Gerakines' nursing license based on her admission to diverting Norco, Percocet, and Fentanyl for self-administration while working as a nurse and her admission to documenting administering drugs, including Fentanyl, to patients and then keeping those drugs for self-administration. These events would constitute grounds for disciplinary action pursuant to ORC 4723.28(B)(8), (13), (16), & (19) and OAC 4723-4-06(G). Ms. Gerakines is entitled to a hearing in this matter if it is requested within 30 days of the last date of publication of this notice. Ms. Gerakines may appear at the hearing in person, by her attorney or by another representative permitted to practice before OBN, or she may present her position, arguments or contentions in writing.
