Pratt, RN, LPN On May 23, 2019, the Ohio Board of Nursing issued a Default Order indefinitely suspending Ms. Pratt's nursing licenses and imposing conditions for reinstatement, based on her failure to submit to a mental or physical examination when directed. On May 24, 2019, the Board mailed the Default Order by certified mail to Ms. Pratt's address of record at that time. The Board was unable to confirm delivery. Ms. Pratt is entitled to a hearing in this matter if it is requested within 30 days of the last date of publication of this notice. Ms. Pratt may appear at the hearing in person, by her attorney or by another representative permitted to practice before the Board, or she may present her position, arguments or contentions in writing.
